VOTER REGISTRATION
General Information The Florida Voter Registration Application is a standard statewide form and can be used by all permanent residents of the state seeking to register or update their voter registration records. In order to register to vote one must:
The registration form is available both in English and Spanish The registration form may be photocopied The form must be used to report change of address (when moving from one Florida county to another), name change, change of party affiliation, and to update signature. An applicant who fails to designate party affiliation will be registered without party affiliation. If the information provided on the form is not true, the applicant may be convicted of a felony of the third degree and fined up to $5,000 and/or imprisoned for up to five years. If an applicant has been convicted of a felony or has been declared mentally incompetent with respect to voting by a court of law, and is unsure whether his/her civil rights have been restored, he/she may call the Supervisor of Elections office at (941) 861-8600. An applicant should fill out his own registration form and must sign it. However, you may help an applicant to fill out a form if he/she needs assistance. The form does require the applicant's signature to be witnessed and the applicant DOES NOT have to show iidentification. Each applicant should be encouraged to print legibly and reminded to sign and date the application form. It is illegal to influence or attempt to influence a person in the free exercise of his/her right to register to vote.
